Wound care is the evaluation and treatment of cuts, ulcers, pressure injuries, and surgical or traumatic wounds that are slow to heal. Our clinicians assess each wound, clean and treat it, choose dressings, and create a plan to support healing and comfort.

We care for most chronic and complex wounds, including diabetic foot ulcers, venous and arterial leg ulcers, pressure injuries, non healing surgical wounds, traumatic wounds, and mixed etiology ulcers. If we are not the right service for a wound, we help guide you to the right level of care.

We provide mobile wound care across California. We see patients in skilled nursing facilities, hospice and home health programs, assisted living or board and care homes, and in select home settings where an agreement is in place.

We typically see adults with chronic or complex wounds who are already under the care of a physician, nurse practitioner, or physician assistant. Our visits are ordered by the treating provider and coordinated with the facility or agency caring for the patient.

Yes. For most settings, a referral or order from a licensed provider is required. If you are a patient or family member, you can call us and we will guide you on how to ask your doctor, hospice, or home health agency to send a referral.

During a visit, our clinician reviews the chart, examines the patient, measures and photographs the wound when appropriate, performs treatment such as debridement if indicated, and applies dressings. We leave clear dressing orders and document the plan so your team knows what to do until the next visit.

Warning signs can include increasing redness, swelling, warmth, worsening pain, foul odor, pus or drainage that changes in color, or fever and chills. If you notice any of these, contact your nurse, doctor, or agency right away. If there is an emergency such as fever and confusion, severe pain, or spreading redness, call 911 or go to the nearest emergency room.

We work with Medicare and select commercial plans. Coverage depends on the patient’s insurance, diagnosis, and setting of care. When you contact us, we review coverage details and let you know what to expect before visits begin.

Provider representatives can visit our “Work with us” page, complete the Service Agreement form, and indicate the settings and counties where they need coverage. Our team sends a Service Agreement for review and signature, then schedules onboarding and sets up an easy referral workflow.
